Overview of Community Assessment and Community Health Nursing Intervention: Community assessment is a systemic way to determine the health status, resources or needs of a population. Community Health Nurses (CHN) assess the community by using the nursing process:

Module 1: CH nurses gather subjective data (i.e. windshield survey and interviewing key informants). Based on this portion of the assignment you will analyze your findings and provide a summary of the key community health issues for your community.

Module 2: Next, you will obtain objective data (Demographic, Education, and Economic date, as well as Morbidly and Mortality rates, Health risk rates – obesity, alcohol use, seat belt use, etc.).  After collection and analysis of the data, Community Health Nursing needs are determined by comparing their community findings to the larger community (city/county, state and nation). Base on those comparisons, you will develop two priority Community Health Nursing diagnoses.

Module 3: Lastly, Community Health nurses develop interventions to address those priority health needs (Community Health Nursing Diagnosis) and indicate methods to evaluate the effectiveness of the interventions proposed.

Overview: Windshield Survey of the Community and Interview Key Community Informants

The purpose of this assignment is for you to obtain a clear understanding of the physical environment, people and services within the community, as well as,to begin to understand the community’s perceptions of their needs. Before beginning this assignment, watch the following video.

This windshield survey was conducted in London,Ohio for a nursing course. This video is only one example of a Windshield Survey.

 You will survey similar items; however, your survey will not be exactly as the example. In this assignment, you will conduct a windshield survey of a geographic area within or near the community where you live. Windshield surveys are effective in raising awareness of issues relating to public health in and around the community. Before you conduct the windshield survey, you may also wish to visit theGoogle Maps website (Links to an external site.). Input the zip code for your selected area. Once the map displays, view the map to determine if there are schools, parks, churches, bus stops, health care facilities, etc. in the vicinity. Note that Google Maps includes both a traditional Map view and a photographic Satellite view of the area.

This assignment is due by Saturday, 23:59 PM of Module 1.

Directions

Locate a community focal/starting point (for example, neighborhood, church, clinic, school, other) in the city or county where you live. Once you select a focal or starting point, you will conduct a survey of the community surrounding the site. Specifically, you will survey a 10-block radius of the facility – 10 blocks north, 10 blocks south, 10 blocks east, and 10 blocks west. If something is not within the area of the community you are surveying (for example, a Hospital), note such and state how far away it is and how people will access it. If you do not see something within the community (for example, homeless people) note that no homeless persons were observed.

Additionally, you will survey two community members about their perceptions of the 2 health and 1 safety needs/issues and 2 strengths of their community.

Windshield Survey Considerations

If you do not use this template, there will be a 10- point  grade penalty ,and you will be required to resubmit within 48 hours,or a zero will be given. In order to do this assignment, you must put aside prior knowledge about your community and gather information as if you were observing places and things for the first time. You will be reporting on only those things you observe through the windshield. Use the information found in the Instruction document to complete this assignment to guide this observation experience.

Use a map of your county to decide what areas you will include in the windshield survey.

Use the attached assignment document to create a list of things to observe.

It is recommended that you request someone else to drive you. This will allow you to focus your attention on what you are observing.

Do not put yourself in danger. If at any point, you feel at risk or threatened, leave the area immediately. Your safety is of utmost importance.

While driving through the various regions, keep detailed notes about what you observe including the different kinds of buildings, businesses, service agencies, and recreational spaces.

Compare and contrast areas by looking at the road conditions, types of housing, people on the streets, rural or urban in nature, etc.

You will complete the tables included in the assignment document with the information that you locate. Your final report should summarize what you find in area you survey.

Community Statistics Assignment

Overview: Statistical Health Data for a Community

In this assignment, you will review and assess statistical data for the community you selected for your community Windshield Survey.  By reviewing demographic, health, crime, income, and educational data, and by comparing local data with city, county, state, and national data, you will be able to determine how well the selected community functions.

Your statistical analysis will address the community in general using the categories noted above. In addition, based on your observations from Module 1 subjective assignment, (windshield survey and key informants) provide Morbidity, Mortality (M&M) and risk data for the health problems you identified in your community, and gather comparative data from the local, city, county, state and national levels specific to those health issues.  For Example 1: if you observed obesity in your community then present obesity rates and health conditions associated with obesity [i.e. CVD, HD, Depression, etc.). These variables may include the following: gender (who does this problem affect the most, men or women); age distribution; ethnicity or racial group(s) mostly-affected; morbidity and mortality rates; any data related to life-expectancy in light of the health issues; is there a geographic area where the health problems mainly exist; is there a relationship between the health issue(s) and environmental health hazards; are these life-threatening health issues?

This type of inquiry in Module 2 Part II will provide the basis for the next paper in this course, the Nursing Intervention paper.  In that assignment, you will develop a community health nursing intervention(s) to address the community health problems you have defined.  There is a progression in problem definition from the Windshield Survey to the Statistical Analysis, to the recommendations for community health nursing interventions.  You will be able to follow the unfolding of this process from problem-identification to recommendations for intervention.  This process requires skills in data-gathering, data analysis, critical thinking, evidence-based nursing, and planning.

Overview:

In Assignment Three, you will analyze and integrate findings of the two previous assignments. These are the Windshield Survey, including the informant interviews, and the Community Health Statistical Analysis. From these sources of information, you will establish a community health nursing care plan.

This assignment is due by Saturday, 23:59 PM of Module 3.

Directions:

The steps you will follow in this assignment are comparable to the development of a Nursing Care Plan. The main difference is that the point-of-impact is a community, an aggregate of people, versus an individual. For this assignment you will analyze and integrate the findings of your windshield survey, information from key informants, and your community health statistical analysis. This analysis and integration will help you establish a community health nursing plan of care.

This assignment requires that you take the following steps:

Introduction: You will state the PURPOSE of your paper. You will also provide a brief synopsis of your community and present two CHN from your observations and the statistical data from the previous two assignments. You will include key supporting evidence for the two CHN diagnoses.

Select ONE Diagnosis/problem based on the two (2) CHN Diagnoses you developed in Week 2 OR a newly developed CHN diagnosis based on Coach feedback from your two (2) CHN diagnoses in Assignment 2.

Discuss the rationale for selecting this problem/Diagnosis.

Provide supporting community and scientific evidence, for example, Healthy People 2020, Center for Disease Control (CDC), American Heart Association (AHA), and others for this diagnosis.

Develop a plan of nursing intervention.Explain the intervention time line. Remember to be realistic when setting the goals and the time line for the various intervention activities to take place.

Describe the Goals (minimum of 2) and objective (2-3 PER goal) of the CHN nursing intervention. Remember, goals are where you want to go. Objectives are how you plan to accomplish the goals.  Goals and objectives need to be realistic.
