In 2005 an explosion rocked the BP Texas City refinery, killing 15 people and injuring 180. The company incurred direct and indirect financial losses on the order of billions of dollars for victims’ compensation as well as significant property damage and loss of production. The internal BP accident investigation and the Chemical Safety Board investigation identified a number of factors that contributed to the accident.

A risk assessment is an important element of health and safety for any business as this will help prevent accidents and serious harm to all concerned. However, a basic knowledge of health and safety is required before a risk assessment can be completed.

This week’s assignment is to build a risk assessment plan for your team project.  The purpose of this plan is to gather background data that you would normally have working for a company.  The plan will address the potential environmental, health, and safety hazards in the chosen industry and assess the risk to your company.  Your team may choose a current or former workplace or can use one of the assigned companies below:

•             Pacific Gas & Electric (PG&E) – California

•             Weyhaueser Logging – Oregon

•             Crown Roofing – Florida

•             Bayway Refinery – New Jersey

Instructions

1.            Summarize a short overview of the general industry (i.e., petroleum refining) using the Standard Industrial Classification (SIC) system.  Identify the Division and Major Group that your company belong to.  https://www.osha.gov/data/sic-manual

2.            Provide the short history of the company andinclude a mission statement, the product or service, and basic information about your company’s leadership team, employees, and location.(1-2 paragraphs).

b.            Research the state and federaloccupational health regulations for your industry.  Hint: There are specific state regulations that will affect your company so be diligent.

c.             Use the Bureau of Labor Statistics (BLS) Injuries, Illnesses, and Fatalities website to identify your occupational hazards, top safety issues and injuries/illnesses.

d.            Research the specific environmental state regulations for where your company is located.  Identify any ecological concerns for your specific area. 

e.            Research your industry for typical environmental issues. These hazards could be effects from past activities or future effects from current or project processes.

f.             Identify any potential receptor(s) – something that could be adversely affected by a contaminant, such as people, an ecological system, property, or a water body.

4.            Prepare report that details your research and follows the IMRD guidelines found in the Week 2 Learning Resources.

Week 3 Assignment  

Lagging indicators are output-oriented, popular among management because they are easy to measure. Unfortunately, they are also harder to influence, since they consist of incidents that already occurred.Leading indicators are a valuable tool that you can use to make measurable and long-lasting improvements to safety and health outcomes in the workplace.

Instructions:

1.            Based on the occupational hazards identified in Week 1, use theBureau of Labor Statistics (BLS)Injuries, Illnesses, and Fatalities website toidentify two (2) possible lagging indicators in your chosen industry.    

Example:  For office workers, tracking the number of lost workdays due to Carpel Tunnel Syndrome is a lagging indicator. 

2.            For each lagging indicator, set a goal that can help improve your safety or health performance. 

Example:  Your investigation reveals that most of the affected office workers have duties that require long hours of typing on a computer.  After looking at their workstations, you notice that the workers rarely take a break, so you set a goal of taking breaks every hour to stretch the hands, wrists and fingers. 

3.            Set a leading indicator based on your goal. 

Example:  The leading indicator in this case would be the frequency of breaks.

4.            Prepare a word document that details your research.  This document must include the research you performed, references used for your assessment and any other pertinent information that supports your assessment.  Include reference citations in APA formatting.

Week 4 Assignment  

Hazard identification is part of the process used to evaluate if any situation, item, thing, etc. may have the potential to cause harm aka “What can go wrong?”. A hazard is any source of potential damage, harm or adverse health effects on something or someone (for example, to people as health effects, to organizations as property or equipment losses, or to the environment). 

This week’s assignment has two parts:  complete a Job Hazard Analysis for a task or process at your team’s chosen workplace and construct a risk matrix for an identified hazard.

Instructions:

Part A (60 points):

1.            Using the provided format in Week 4 Reading, follow the steps to complete a Job Hazard Analysis for a task or process at your team’s chosen workplace.

a.            Select the job for analysis.

b.            Break down the job into a sequence of steps.

c.             Identify the hazards and hazard scenarios for each step.

d.            Identify the control measures or prevention measures for each step.

e.            Identify the person responsible for implementing the control or prevention measures.

Week 5 Assignment  

Failure events in Fault Tree Analysis are described as risk events. Risk events can be conceived by asking the question “What can go wrong?”.  This top-down approach starts by supposing that an accident takes place. It then considers the possible direct causes that could lead to this accident. Next it looks for the origins of these causes. Finally, it looks for ways to avoid these origins and causes. The resulting diagram resembles a tree, thus the name.

This week’s assignment is to construct a Fault Tree Analysis for a basic eventin your workplace.  For detailed guidance, refer to Week 5 reading, and Chapter 11.5 of  Hazard Analysis Techniques for System Safetyfound in Week 5 Learning Resources.

Instructions:

1.            Using the provided format, follow the steps to complete a Fault Tree Analysis for a basic event at your team’s chosen workplace.Document your diagram in  PowerPoint or any other similar professional product.

a)            Define the system to be analyzed.

b)            Describe the top-level risk event (e.g., system failure).

c)            Describe intermediate events (e.g., subsystems failure) that contribute to the top-level event and connect them with logic gates as appropriate.

d)            Identify critical events and pathways to failure.

2.            Prepare a word document that details your research.  This document must include the research you performed, references used for your assessment and any other pertinent information that supports your assessment.  Include reference citations in APA formatting.

Week 6 Assignment  

Upon completing the hazard analysis, an evaluation is performed to determine the significance of each risk, which risks are at acceptable levels and which risks need further treatment to get them to acceptable levels.

This week’s assignment is to prioritize the risks analyzed in Weeks 4 and 5 then develop a risk treatment strategy. 

Instructions:

1.            Select a method to prioritize your risks and document using a risk register.  As a minimum, your register should include the risk identified, the severity of the risk, likelihood of the risk occurring and the overall risk score.

2.            After identifying and scoring the risk, identify the treatment to handle your top priority risk.  Document your steps taken for the risk treatment chosen and provide justification for your choices.

3.            This document must include the research you performed, references used for your assessment and any other pertinent information that supports your assessment.  Include reference citations in APA formatting.
