AMU Nursing Advance Directive in Medical Care Facilities Discussion

Consider this scenario: A current School of Nursing faculty member recalls a recent post surgical scenario…

“As a 58 yr old pt with several ailments, I opted to have back surgery in Sept of 2020. I brought my Advanced Directive to the Admissions Clerk, who placed it in my file.
While in the recovery room (PACU), my throat and right lung collapsed causing severe respiratory compromise. I recall everyone becoming very excited and calling for other staff, saying my left lung and heart were being ‘crushed’ as well.
Long story short, I was intubated and had a chest tube inserted in a brisk attempt at refilling my lung. I remember saying, just after the airway was removed, that they weren’t supposed to do all of that- I had an advanced directive. A male voice asked, ‘Does she?’, and a nurse responded, ‘Yes. She does.’
The following evening, suction was disconnected from the chest tube, and within a few hours, the lung collapsed again! Suction was reconnected to re expand the lung as soon as this issue was identified.”

Discuss the legal and ethical principles in this scenario. If you were the nurse in charge, what would you do?
