Evaluate your Field Placement Work

Instructions

This week, you will complete your evaluation of your field placement. You will have been working with your field instructor to demonstrate the behaviors within your agency.   This survey helps determine your level of mastery of the nine competences and the 29 behaviors that measures these competencies. For areas where you have not received a score of 3 or better you will work with your field instructor to create a plan on how you will reach competency on these areas during the second half of your field placement. 

This evaluation sets up the goals for the second half of your practicum experience.  For any items where you were not able to demonstrate competence you will work with your field instructor to ensure you will have opportunities in the second half of your practicum to meet competency.
