Identify if your plan will focus on government, nonprofit, or public
health management

Clearly state which sector your plan will involve. State why you
selected a particular sector. If it’s a public health facility, specify the
type, e.g., cancer center, PPO, behavioral health center/hospital, hospice,
etc.. If it’s a government entity, state the location, whether city, state,
or federal, as well as the department, e.g., education, corrections. If a
nonprofit, state the type of organization and services to be provided,
location and potential name for the organization.

P-Plan, as well as a brief description

Needs Assessment

Identify the unmet needs that you want to address with your P-Plan and
program. Refer to authoritative surveys, external sources, demographic
information, pertinent statistics, interviews of noted experts in your
P-Plan. The needs assessment should stand by itself so that it clearly and
convincingly documents the needs your program intends to meet.

Identification of stakeholders

List the stakeholders, their profession or job, why they are a
stakeholder, their qualifications to be a stakeholder, if pertinent, the role
they will play, and how the program will benefit from the stakeholder.

Demonstrates ability to properly make a professional presentation

Introduction: Did the introduction
lead into the presentation well? Was it organized and brief? Friendly,
enthusiastic and attention getting? Professional?

Idea Presentation: Was the idea for
the project made clear during the presentation? Could the audience “get
it” from the presentation alone? Were the presentation slides well done?
No errors in grammar or spelling? Visible, legible and appropriate to the
topics? Verbal Presentation Quality: Did everyone present? Was the quality
