INTRODUCTION

As a nurse, you must be able to perform a complete physical assessment on a patient. This skill is the foundation of nursing practice. For this task, you will record yourself completing a physical health assessment on a family member or friend (do not use a patient in a healthcare setting) using the  video recording system.

The “Video Health Assessment Success Guide” is provided in the File Attachments section of this task to offer additional course instructor support with the task directions.

If you have any problems accessing  or the Applied Health Assessment – folder, please contact Assessment Services at assesmentservices@.edu. It will take up to two business days to receive your   recording permission once you have referred for the course.

You will submit your recording for evaluation. Additionally, you will be required to document the assessment, including primary assessment findings, using the “Physical Assessment Documentation Form (PADF)” located in weblinks. You will also complete an analysis of the assessment findings. Note: Cueing or receiving assessment directions from the patient or any other party is not allowed and is considered an originality concern.

To prepare for this task, you should review the methods for conducting a physical assessment in your learning resources in order to familiarize yourself with the components of a physical assessment. As you review your learning resources, pay careful attention to assessment techniques and methods. You will be evaluated on proper techniques as you perform the physical assessment. You should practice conducting the assessment using the PADF located in weblinks. This assessment form will be used to evaluate your skills performing the health assessment in the recording that you will submit for this task. Directions on how to use the PADF are in the attachments section.

Before you are ready to begin recording your assessment for submission, please have the friend or family member who is acting as your patient complete and sign the attached “Nursing Program Audio/Video Recording Consent and Release Agreement (Patient).” Your patient must be an adult who has sufficient capacity to provide informed consent based on the rules and regulations as defined by your state of residence and is 18 years of age or older. For example, individuals from the following groups will not have the ability to provide informed consent: minors (particularly children with special needs), cognitively impaired persons regardless of age, and wards of the state—with the exception of emancipated minors. The patient’s signature has to be handwritten. Once the consent form is signed by the patient, it should be either scanned and uploaded in EMA or photographed and uploaded as a .jpg, .png, .bmp, .gif, or any other image file format. The consent form must be legible

To submit this task, you will upload the recording to the  folder titled “Video Health Assessment – EEM1|C349 [assignments].” Once the recording has been uploaded and saved into ’s system, you will retrieve the URL of the recording from  and submit it, along with the remaining task requirements, in EMA for grading.

You will also need to instruct your patient to dress in shorts and a tank top so evaluators can see you performing the correct assessment techniques. You will not need to lift the patient’s clothing for any of the assessment. Speak clearly and perform all of the required skills listed on the PADF. Verbalize your assessment as you move through to ensure the evaluator is aware of what you are assessing. This is especially important during inspection.

REQUIREMENTS

Your submission must be your original work. No more than a combined total of 30% of a submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. An originality report is provided when you submit your task that can be used as a guide.

You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.

A.  Submit the completed copy (including the date and the patient’s name, address, date of birth or age, and handwritten signature) of the attached “Nursing Program Audio/Video Recording Consent and Release Agreement (Patient)” for the volunteer patient for this assessment.

Note: Your patient must be an adult who has sufficient capacity to provide informed consent based on the rules and regulations as defined by your state of residence. To ensure compliance with HIPAA regulations, do not use a patient in a healthcare setting for this assessment.

B.  Provide a recording of yourself completing a physical assessment using the  system by doing the following:

Note: Before you begin recording, please verify that both you and your patient can be seen in the viewing field.

Note: Videos must be submitted through . Submitting links from outside sources will result in the submission being returned without evaluation.

Note: The assessment should be captured in the video. If you report a finding in your documentation, it should be represented in the video submission.

1.  Provide a functional URL for your  recording that includes a clear audible verbalization of each step in the assessment process.

2.  Demonstrate that you can successfully perform the assessment techniques for each assessment item listed in section 1 of the PADF by achieving a minimum score of 9 out of 11.

3.  Demonstrate that you can successfully perform the assessment techniques for each assessment item listed in section 2 of the PADF by achieving a minimum score of 16 out of 19.

4.  Demonstrate that you can successfully perform the assessment techniques for each assessment item listed in section 3 of the PADF by achieving a minimum score of 10 out of 12.

5.  Demonstrate that you can successfully perform the assessment techniques for each assessment item listed in section 4 of the PADF by achieving a minimum score of 16 out of 19.

Note: You are starting at zero and working to a passing score in each section. Assess both sides of the body for comparison where appropriate.

C.  Submit a completed copy of the PADF that documents the findings of the physical assessment you performed.

Note: If you did not complete a technique during your video, you may leave that section of the PADF unpopulated.

Note: Use only the linked version of the Physical Assessment Documentation Form (PADF).

1.  Analyze (suggested length of 1–2 paragraphs) the primary findings of the physical assessment you performed.

Note: You might consider the following for your analysis: What is your overall impression of the patient’s health? Provide data and details to support your impression. Describe any abnormal findings. Can you correlate these findings to pertinent patient health history? You can add this to the appropriate box at the end of the PADF or submit it as a separate file attachment.

D.  Acknowledge sources, using in-text citations and references, for content that is quoted, paraphrased, or summarized.

E.  Demonstrate professional communication in the written content of your submission.
